Existing patients – Schedule or reschedule an appointment – Please call your provider’s office to schedule or reschedule an appointment.
Get a referral – Visit the patient portal to send messages to your provider to request a referral. If you do not have a patient portal account, call your provider’s office to request a referral.
New patients – Schedule an appointment – Identify a clinic or provider using our find a doctor or find a clinic tools and call to schedule an appointment with the provider of your choice.
Prescription refill – Contact your pharmacy to request a refill. You can also request a prescription refill by visiting the patient portal. If you do not have a patient portal account, call your provider’s office to request a prescription refill.
Request medical records – Visit the patient portal to send messages requesting medical records. If you do not have a patient portal account, call your provider’s office.
Sign up for the patient portal – If you are an existing patient, you will have received an e-mail with login instructions. If you did not receive this e-mail or cannot find it, call your provider’s office to have a login e-mail resent.
View lab or imaging results – Log on to the patient portal to view lab or imaging results. If you do not receive your lab or imaging results, send a message to your provider. If you do not have a patient portal account, call your provider’s office.
