CareCoach Connect is a medical care support service available to you as a UnitedHealthcare Medicare Advantage plan member.

The goal of CareCoach Connect is to help you get better care and make it easy for you to do so by coming to your home.

During the visit, the CareCoach Connection clinician will talk with you about your medications, your health, any problems or issues you are experiencing and any serious illness(es) you may have. They will gather all your health information and make it available for you to share with your primary care doctor and any specialists you may be seeing. That way, everyone has a clear picture of your current state and can help you with your health.

If a home visit doesn’t work for you, you can choose an online video visit instead. A home or online visit takes about an hour.
